Lauwers-Early gastric neoplasms

چکیده

Despite a marked decline in incidence in the West and some decrease in the East, gastric cancer remains a significant cause of morbidity and cancer related deaths worldwide. The prevalence of gastric cancer is closely related to prevalence of Helicobacter pylori infection and shows wide geographic variation. Subsequent chronic gastritis, atrophy, and intestinal metaplasia are lesions that confer a high risk for the development of gastric cancer, while gastric dysplasia, the penultimate stage of the carcinogenetic cascade, is a direct neoplastic precursor lesion.2, 4, 14-16, 18, 22, 36, 41, 60, 64 Evidence for gastric dysplasia as a direct precursor of adenocarcinoma stems from observational studies reporting high-grade dysplasia (HGD) in close proximity to 40-100% of early gastric cancers and 5-80% of advanced adenocarcinomas.48, 61, 95 Moreover, dysplasia is also a marker of increased risk for cancer elsewhere in the gastric mucosa.

[Pathologic diagnosis of gastric epithelial neoplasia].

Gastric epithelial neoplasia is a very common disease entity in Korea, encompassing gastric adenoma and adenocarcinoma. There are still discrepancies in pathologic diagnosis of gastric epithelial neoplasia between Western and Japanese pathologists after Vienna consensus classification. With increasing use of endoscopic therapy such as endoscopic mucosal resection and endoscopic submucosal disse...

Gastric extremely well-differentiated intestinal-type adenocarcinoma: a challenging lesion to achieve complete endoscopic resection.

Extremely well-differentiated tubular adenocarcinomas (EWDAs) of the stomach are characterized by surface maturation and their mimicking of intestinal metaplasia. Endoscopically, intramucosal EWDAs are frequently ill defined with indistinct borders due to the pallor of the neoplastic mucosa and the lack of contrast against the background atrophic and metaplastic mucosa. We evaluated the effecti...

Poorly differentiated component in gastric pinch biopsies predicts submucosal invasion

BACKGROUND Endoscopic resection has become standard therapy for selected patients with early gastric carcinoma (EGC). However, the preoperative diagnostic accuracy for excluding submucosal (SM) invasion is not precise. Moreover, histologic features predicting SM invasion in gastric carcinomas (SMiGC) have not been studied extensively. METHODS Pre-treatment gastric biopsies from 60 patients wi...

5-Year Survival in Gastric Adenocarcinoma with Epithelial and Stromal Versican Expression

Background & Objective: Gastric cancer is the second most frequent cause of cancer death worldwide, despite differences in incidence around the world. The majority of gastric cancer cases concern gastric adenocarcinoma, which has a fairly high 5-year survival rate when coupled with early-stage diagnosis. Versican, a member of the aggregating chondroitin sulfate proteoglycans fa...

Pyloric Gland Adenoma in Lynch Syndrome

The prevalence of gastric cancer associated with Lynch syndrome (LS) is highly variable, and the underlying histologic pathway or molecular mechanisms remain unclear. From 1995 to 2012, 15 patients had been treated for both gastric and colonic adenocarcinomas and diagnosed as LS. In all cases, pathologic review, immunohistochemical analysis for mismatch-repair proteins, and microsatellite insta...
